Student Enrollment

Below are the forms you need to enroll your child at any school in Malakoff.  Please fill out these forms and bring them along with immunization records, certified birth certificate, social security card, proof of residency, and withdrawal papers from the school your child is leaving.  We look forward to serving you and your children.

Malakoff ISD does offer free out of district enrollment upon acceptance of the principal and superintendent.

Electronic Enrollment Form

Manual Enrollment Form
Print and fill out by hand.
Student Transfer Request
Fill out this form and bring it to the appropriate principal. The building principal and superintendent must approve the request before the student is enrolled. Approval is based on the behavior and academic performance of the student as well as the current enrollment in the grade level the student is requesting to enter.
